The on-line application allows you to submit the required application materials securely using the internet. You will be required to select the application type (summer camp or ESL), create a user account and at the end of the process submit the application fee via credit card. The application does not need to be completed in one sitting. You can log into and out of the application system allowing you to finish the application and submit the required materials at your convenience.

To complete a classic paper and pencil summer camp / ESL application complete the following documents and send them to us via mail:

Summer Camp Application Fee: $150

Report of Medical History (requires physician's signature)	English	Spanish

Consent & Insurance Form	English	Spanish

Complete Immunization History (Texas state law requires complete history)

Medical Addendum Form	English	Spanish

Copy of Birth Certificate
